Most people will live in their own homes all of their lives. Only a
relatively small percentage of people will need to move into an aged care facility.
If you believe that you or your relative should consider moving to an aged care facility
where you will receive 24 hour care you may choose which aged care service is suitable to
your care needs. The avenues provided will assist you in understanding the move to
residential care.

Aged Care Assessment Teams (ACATs) have been established by the Commonwealth Government
to help you decide which services will best meet your needs. The ACAT team is usually
based at a hospital or community centre and may include a doctor, nurse, social worker,
physiotherapist or occupational therapist.
It is best to talk to your own doctor or local hospital first to see if you need to be referred
to an ACAT.
Once you have made an appointment, a member of your local ACAT team will visit you in your home
or in hospital to make an assessment of your needs.
